Chronic Disease Management

We will arrange twice yearly reviews for all patients over 18 with the following conditions: 

• Asthma

• Chronic lung disease (COPD)

• Ischaemic heart disease

• Heart failure

• Atrial fibrillation

• Type 2 diabetes

This is provided free of charge in conjunction with the HSE. Contact reception if you have one of the above conditions and have not had your review in the past 6 months.
